If you do, please take a minute to review how your benefits work for injectable and infusion drugs. We cover injectable drugs under two different parts of your BlueChoice HealthPlan benefits.
We cover seven specific drugs under your prescription drug coverage — Byetta, epinephrine, glucagon, Imitrex, insulin, Lovenox, and Symlin. You will pay your regular pharmacy copayments for these drugs through a retail pharmacy, or mail order.
All covered injectable and infusion drugs are managed through your medical coverage. Please remember, you have a specialty pharmaceutical copayment for each administration of covered specialty infusion medication. Also, there is a single specialty copayment for each month's supply of self-injectable medications.
There are several injectable drugs that require prior authorization. Your doctor's office will get the authorization by contacting ICORE at (800) 327-8648. ICORE Healthcare is an independent company that provides specialty pharmacy services. On behalf of BlueChoice HealthPlan, ICORE assists in the administration of the program. Also, your doctor will usually make arrangements with ICORE Healthcare for you to get any self-administered or home-use injectables (including antibiotics), even if they don't require prior authorization.
Oral specialty medications may be subject to a specialty pharmaceutical copayment per month’s supply. Members should consult their Schedule of Benefits for copayment amounts. Members must fill these oral medications through a retail pharmacy. They are not eligible for mail order. Your doctor will contact Caremark at 1-800-294-5979 for any oral specialty drugs that require prior authorization. On behalf of BlueChoice HealthPlan, Caremark assists in the administration of the program. Caremark is an independent company that provides pharmacy benefits management.
