Wellness Incentive Program

BlueChoice HealthPlan’s NEW wellness incentive program, FOCUSfwd, is here! Your employees can get healthy (or healthier) today and have fun while doing it! This program offers tangible rewards to promote, motivate and incentivize your employees to take part in improving and monitoring their health. FOCUSfwd is available at no additional cost to your employees with BlueChoice® coverage and their covered spouses (age 18 and older).

Why Change?
We reviewed our programs and realized they weren’t heavily utilized. We conducted extensive research to create a program that would benefit all of your employees. Our goal is to help your employees get healthy and stay healthy, which will lead to decreased medical costs and claims.

How Does It Work?

FOCUSfwd gives them the chance to win gift cards JUST for making healthy choices! There are winners EVERY WEEK. The more they participate, the better their chances are of winning the BIG GRAND PRIZE of $5,000!

They can get started by:
  • Logging into their My Health Toolkit® account
  • Clicking on the Health and Wellness tab
  • Then clicking on the FOCUSfwd Incentive Program link. 
If they don't already have a My Health Toolkit, it takes just a few minutes to create one. They'll need to have their member ID card available. 

 















BlueChoice handles the FOCUSfwd Wellness Incentive Program administration of the program by: 

  • Providing credit for the activities and mailing gift cards
  • Providing materials to promote the program

 *This program is only available to subscribers and their spouse within applicable employer groups.Your employee may call the Customer Service number located on the back of their member ID cards to confirm if this program is available to them. Or you may contact your Marketing Representative to confirm if this benefit is available to your employer group.
