MyChoice Individual Health Coverage

New - Family coverage available June 1!

Effective June 1, BlueChoice HealthPlan will offer family coverage through MyChoice Individual Health Coverage. MyChoice also offers coverage to South Carolina residents ages 19 through 64 and a half with several benefit plans from which to choose. To apply for family coverage, please fill out this application. To apply for individual coverage, you can apply online. Our online application will be available for family coverage soon.

  • Primary Care office visits with comprehensive copayments
  • Routine preventive office services 
  • Prescription drug card — copayment for generic drugs, preferred drugs and non-preferred drugs
  • Vision care — free annual eye exam
  • Dental care — $30 towards cleaning (one per benefit period) and $20 towards routine exam (one per benefit period)
