You can request some authorizations directly through our Web site. Please review the Ancillary Services Prior Authorization Required list to find out if you can request the service your patient requires through the Web.
Please use the Prior Authorization Request Form to request all other prior authorization-required services. Print, complete and fax the form as indicated.
Specialty Pharmacy: Beginning August 1, 2009, all Specialty Pharmacy services will be provided through ICORE Healthcare. ICORE Healthcare is an independent company that provides specialty pharmacy services. On behalf of BlueChoice HealthPlan, ICORE assists in the administration of the program.
