Electronic Funds Transfer

BlueCross BlueShield of South Carolina and BlueChoice HealthPlan have been accepting enrollment for electronic funds transfer (EFT) since August 1, 2002.

Please complete the information below to begin the enrollment process. Once we receive the completed information, we will send you a detailed enrollment package which will require you to provide detailed financial information. The entire enrollment process should take from 30 to 45 days, which includes time for testing with your bank.

  • Facility Name
  • Provider Number (9 digit only)
  • Federal Tax ID (Include suffixes)
  • Contact Name
  • Telephone Number
  • Mailing Address
