End Stage Renal Disease

Patients who suffer from end stage renal disease are going through a difficult time. We can help make the treatment process easier with a special program, especially for members on dialysis.

A community-based Registered Nurse with experience in renal management and dialysis will help coordinate all care. She'll meet with the member and his or her family. She will work with dialysis providers and nephrologists. With this program, we aim to reduce complications and support the member to a successful transplant.
