Companion Care Solutions Renal Case Management and Oncology Case Management Programs

CCS is a division of Companion Benefit Alternatives. It is a health care advocacy program that pairs you with a dedicated case manager who can help you get the information and support you need to make important health decisions.

What can case managers do?
  • Coordinate care management through streamlined communication and condition management.
  • Improve quality of care through early intervention and education.
  • Improve member and provider satisfaction.
  • Coordinate care among all your providers. These might include a primary care physician, nephrologist, dialysis center or oncologist.
  • Provide support and education.
  • Make sure you get counseling about issues such as nutrition to improve your quality of life.
  • Assist you in managing the costs of your condition by helping you avoid hospitalization when possible.
  • Assist members and their health care team by creating a survivorship plan when cancer treatment is complete.
Renal Case Management

Chronic kidney disease (CKD) is a difficult disease that affects hundreds of thousands of Americans. CCS offers a program that can help coordinate care and offer support. CKD means your kidneys have become damaged over time. It progresses when the kidneys stop working and become unable to do their job of removing waste products and excess water from your body and helping to maintain your blood pressure. CKD is a progressive disease. It can be manageable for up to 20 years.

End-stage renal disease is the fifth and last stage of chronic kidney disease. At this stage, your kidneys will need extra help from dialysis or a kidney transplant. Treatments like these can help you live for many more years. Coping with kidney failure can be complicated. You will need changes in your lifestyle, medications and diet. That’s why you might consider the Renal Case Management program for extra help and support.

Oncology Case Management

About 1 in 3 people will be diagnosed with cancer in their lifetimes. Receiving a cancer diagnosis can be devastating and costly. The National Cancer Institute estimates cancer-related direct medical costs in the U.S. were $183 billion in 2015 and are projected to increase to $246 billion by 2030.

The CCS Oncology Case Management program offers claims-based specialty case management on a national scale. We use a proactive, member-centered approach focused on intensive education, care coordination and member support across the health care delivery system and throughout the life cycle of the disease. This innovative approach helps improve clinical outcomes and reduce costs.

We pair each member with a personal case manager, a registered nurse with experience in oncology. Case managers partner with members, helping to coordinate care and improve the use of the medical management tools and resources available.
