Credentialing and Provider File Update
If you’ve submitted a credentialing application or other provider file update request (Request to Add or Terminate Practitioner Affiliation, Change of Address Form, NPI Provider Notification Form, etc.), and are in need of a status update, please contact your BlueCross BlueShield and BlueChoice HealthPlan Provider Relations and Education Advocate. Provider Education Advocates are designated to providers based on the county in which they are located. If you are not familiar with who your representative is, you can view the county list and the corresponding advocates here.
Your Education Advocate will notify you of the status of your file and if additional information is needed to complete it. If additional information is needed, please provide that as quickly as possible so your file can be processed. The Provider Enrollment process cannot be completed until all requested information is received and reviewed.
