1095-B Forms Are Just a Click Away

BlueChoice HealthPlan of South Carolina and BlueCross BlueShield of South Carolina are making it easier for groups and members with fully insured coverage to access their 2020 1095-B tax forms. The 1095-B form (Health Coverage) is sent to individuals by the insurer to report minimum essential coverage. The form details the type of coverage, the months of the year the coverage was provided and the names of those covered by the plan.

Forms will only be available through My Health Toolkit® beginning in early February 2021. Members will not receive a mailed copy of their 1095-B tax forms this year. We are notifying groups and members of this change.

Visit www.BlueChoiceSC.com to log in to My Health Toolkit.

If a member does not have a My Health Toolkit account, they can create one by clicking the “Register Now” button on the www.BlueChoiceSC.com homepage and following the prompts.

Visit www.SouthCarolinaBlues.com to log in to your My Health Toolkit account.

Member Questions and Information:
Members can call the customer service number located on the back of their ID card for more information regarding their 1095-B tax form.
