We offer the Great Expectations® NICU Case Management program to educate and support the parents of infants with certain medical needs or conditions. This includes, but is not limited to, complications associated with premature birth, congenital birth defects, seizures, cystic fibrosis and genetic disorders. We provide this value-added service at no charge just for being a member of BlueChoice HealthPlan. 

With NICU Case Management, everyone involved in your infant’s health care works as a team. This includes you, your family, your doctors and other health care staff, and a BlueChoice® case manager. An experienced registered nurse case manager partners with you to help you navigate through the complicated health care system. Our team of highly skilled NICU case managers includes nurses who typically have more than five years of maternal-neonatal experience.

There are many benefits to participating: 

  • Care management from a clinically experienced, certified nurse case manager. 
  • Educational materials. 
  • Referrals to community and/or professional resources. 
  • Helping members use and understand their pharmacy and medical benefits. 
  • Ongoing communication and coordination with all involved doctors, therapists, home health nurses and social workers. 

We identify members who may qualify for NICU case management. Doctors can also refer their patients who are BlueChoice members, or you can request us to evaluate your infant for participation in the program. Please call us at 855-838-5897 to make this request. A case manager will review your information to see if your infant qualifies for our NICU case management program.
