My Health Planner — A Digital Medical Management App Connecting Patients to Care My Health Planner — A Digital Medical Management App Connecting Patients to Care My Health Planner is a new digital medical management app available to all BlueCross BlueShield of South Carolina and BlueChoice® HealthPlan members identified for case management services as well as those who are identified as high or medium risk disease management (DM). Helping patients achieve healthier outcomes is our number one goal for patients needing help managing their health. Whether recovering from a surgery, dealing with a chronic condition, or experiencing a high-risk pregnancy, there is a care management program designed to help enhance the care you are providing as well as help patients manage their health effectively. Members in the program have the option to connect with an assigned care manager by phone, or through My Health Planner, powered by Wellframe, an independent company providing services on behalf of BlueCross and BlueChoice®. Through the My Health Planner app, members will have access to: In-app, secure chat with a care manager Customized notifications for medications, doctors’ appointments and other daily tasks. Checklist – daily to-do list and personalized educational content Library of articles, pictures, and videos Case Management Programs Case management empowers, educates, collaborates and coordinates care to promote quality services and optimal outcomes for our members. We strive to provide holistic support and care coordination to improve our members’ health and well-being, while enhancing both the member and provider experience. Case managers serve as health care advocates to: Coordinate and integrate care across the multispecialty, multi-setting care continuum. Increase member and provider satisfaction through appropriate, comprehensive medical management. Improve quality of care and adherence to treatment plan through early intervention, education and intensive care coordination. Case management may be appropriate for those who experience: Frequent hospitalizations. Long-term or life-threatening illness or injury. Extensive home health care. Effects of traumatic injury. An illness such as end-stage renal disease that requires intensive, costly dialysis treatment or kidney transplant. Health Coaching The health coaching program helps patients meet their specific wellness goals. Whether that is to reduce their risk for illness, better manage a health condition or provide extensive and targeted support for a more complex health problem, our health coaches work with members to inspire healthy living and overall well-being. Our health coaches use motivational interviewing and mindfulness to guide members through the stages of change. This process helps them clarify their goals, explore options to attain them, enhance their personal accountability, and realize the benefits of change for their health and quality of life. Through this comprehensive, mindful approach, we help improve treatment plan adherence and improved health outcomes. Chronic condition coaching programs include: Asthma (pediatric and adult) Attention deficit hyperactivity disorder (ADHD) for adults Bipolar disorder Chronic obstructive pulmonary disease Depression Diabetes (adult and pediatric) Heart disease Heart failure High blood pressure High cholesterol Metabolic health Migraine Recovery support for substance use We also offer healthy lifestyle coaching for: Back care Stress management Tobacco cessation Weight management (pediatric and adult) Maternity Care The maternity care program provides extra support and encouragement throughout the member’s pregnancy and postpartum period. The program helps guide members through the challenges they may face between office visits and is designed to: Help foster a better maternal health experience for the best possible outcome Educate expectant mothers from the first trimester through delivery and postpartum Encourage healthier outcomes for mothers and babies through one-on-one support, tools and targeted educational content. Convenient, Secure Support Our goal is to meet members where they are by providing targeted, personalized support through multiple channels. Members can reach us by phone, Monday through Thursday, from 8:30 a.m. to 8 p.m. and Fridays from 8:30 a.m. to 5 p.m. We also offer services on Saturdays, by appointment only. Our care management program does not affect patient’s claims payments or benefits. They may opt out at any time.
